a. A standard appointment costs $90 -$220 depending on what tests and how much time is utilized.
b. Print and fill out the Initial Intake Form, read and sign the Informed Consent (submit through the website form)
c. Either call or submit the form online to be contacted so we can arrange an appointment or simply book online then return to this page for the intake forms below. Download the word document and fill it out in word and email it back or print it and bring it with you to our appointment if you are seeing us in-office.
